47603 Fix Scripts

I have 1 site that needs to be addressed and added [login to view URL] previous programmer doesn't have the time and I need to add these lines immediately. 1. The first one is adding 2 additional lines to a form and adding validation to the form . When the user completes the form, it has to be added to preview page and then submit into the mysql.
