I would like someone to setup a MySQL in my GoDaddy account and then link a Wufoo form that I have created to automatically enter the data into the database upon entry. I would also like to have the Wufoo form embedded into my site (use Go Daddy Website builder). The code/API is already written from Wufoo it would just have to be loaded and tested etc.

A Wufoo Form that will auto fill a editable PDF form. We do Chinese visas, and the embassy has a China visa application via PDF for people to fill out. Many customers fill out the application wrong. I would like to have wufoo form that customers can fill out, and depending on what they fill out will allow them to the next question, ultimately ending in the perfect completed Chinese visa PDF application. Some NON U.S countries need additional documents required in addition to the PDF application, so depending on what country the customer passport is from, the completed application will produce a check list of additional requirements needed when the customer prints out. When he first did it, he made custom looking forms that validated with the Wufoo backend; however, Wufoo seems to have since made a bunch of changes to their form backend and it no longer validates. I need to setup Installment Plans and below are the instructions: Every time you get an webhook for the user, you can increment a counter on your side to track the total amount paid by the customer. I have a website that uses mysql and is not working properly. Pages are built in PHP and the basic logic is that it takes entries from several forms and using the wufoo APi, these entries get populated into a mysql database and the database entries are rendered on the site with certain additional logic for sorting, filtering, etc. The solution worked previously until an issue with the database happened and the database was lost. It had to be recreated and I believe that the issues are because the database was not recreated properly. The primary logic works still (Wufoo entries make it into the database and render properly on the site, but certain features like sorting, assigning a logo image to each entry do not work.
