Need to configure checkout page with Woocommerce on Wordpress website

Hello everyone,
My client has a Wordpress website with Woocommerce already installed. I need to someone to configure woocommerce with the checkout pages of our wordpress website.
-There will be no merchant account integration
-My client only wants the customers to see checkout page with total balance and click "Send"
-The card will not process, information will be sent to admin via email
The checkout page should be 1 page only and contain the following information:
1. First Name
2. Last Name
3. Street Address
4. City
5. State
6. Zip Code
7. Choose a meal plan: (Drop down box options): OPTION 1 ( $109.99 or $79.99), OPTION 2 ($119.99 or $89.99), OPTION 3 ($104.99), OPTION 4 ($99.99)
8. Would you like cookies or cold pressed juice? OPTION 1 ($6.95), OPTION 2 ($10)
9. Do you have a guard gate in front of your community? (Drop down box options): OPTION 1 (YES), OPTION 2 (NO)
10. (Check box) By clicking the check box to the left, you agree to have your card charged for the total amount below.
*If you want to discontinue the meal plan service, you must cancel by Thursday at 3pm of every week. (this text should be added at bottom of page)
NOTE- The total balance should include a fee of $5 for shipping to every order

Hello Sir, I have read your requirements in full. Woocommerce provides a checkout page and it needs to be modified by adding the fields you mentioned. I have done this job and have the codes with me as how to handle this task.
A small query: You said at the start of the project description : -- The card will not process, information will be sent to admin via email and also at the end of the description it is said: -- 10. (Check box) By clicking the check box to the left, you agree to have your card charged for the total amount below.
So kindly let me know if you need the card processing facility or not. Woocommerce by default have paypal payment gateway integrated into it. Also there is mijireh checkout available for new versions. Kindly clarify how you like to go with this part.
