Convert a Template to a Website

I am looking for someone to design a questionnaire used for medical/clinical questions. The system issues code like copied at the bottom. Your design is saved as .css file and you use HTML and CSS. You will need to design some icons, at least 3 of them in about 5-6 colors We eventually add header info etc. Use of questionnaire tool: This application allows e.g. patients to fill a questionnaire via ipad. The questionnaire is prepared by e.g. a doctor and covers e.g. a study. The functionality is given by us. Differences of selected item, not selected item currently active/passive, enabled/ desabled, initialized, reset parent-child dependencies/leveling e.g. pyramiding question turns into mandatory field at a lower level (what makes all the question mandatory of course) are to be displayed in order to guide the person who fills (answers) the questionnaire. The questionnaires can be very long, be one more than one page, be filled on tablet, desktop, etc. have lots of reply options Icons like checkbox, radio button : (green, blue, red, orange, black, grey; each color representing one status) possibly further icons for page complete / incomplete questionnaire complete / incomplete Your job would be to change this "developer-styled Reply-questionnaire form" into a good looking visual easy and intuitive to operate (form).
