249866 Form added to website

I have a 2 page website. Right now the form page that collects names etc looks nothing like the main page. I need to have the following done: I need the front page changed so that instead of having a button that says "Click here" and having them brought to the form page, I need that to be an email field. Once they submit their email, I want the form page to - look similar in theme to the front page - use a 3rd party form service that will store the information online and allow me to export it to a spreadsheet easily. I'll pay for the 3rd party service (I think there are monthly services available out there) - just need someone to inpkement everything. PM me for the actual website and any other questions.
