Website form (order procedure)

Question form - Order procedure We are developing a web page where we need to collect information from potential customers through several forms (order procedures). In the forms the customers will be ask several questions (5-10) and in the end be able to review their answers and be able to pay for the service that they, through, the form is buying. Payment should be done through e.g. PayPal standard. We want someone to make One of these forms and upon a successful job be able to complete the rest of the forms in a similar way. So if the job is successful there will be more job. We have the web page where the form is to be situated, we have a style guide that should be followed, and we already have the questions and all other information for the actual form. You will complete a working form that looks good in two weeks time. As the initial job is very small (only one form), we won’t accept any big offers for the job. If successful there will be at least 8 forms to do at this point of time. The rest of the forms should then look exactly the same as the first with the exception of the questions asked in them. Your chance of being accepted as a bidder would be exceptionally higher if some reference work of forms made can be shown, or examples of how you would do it. Good luck!
