Access form for company intranet

Dear all, I am looking for a simple but good looking form where our field force can enter their monthly driven miles. The form should be accessed through the intranet and only allow access to a certain user group which is pre determined through a list of employee numbers. Each employee should only see his own data but the administrator must be able to see all details, be able to add people to the list and extract data. I want to be able to add our logo to the forms etc on a later stage so I need to have this option as well. I can prepare something on my own but don't have the time to make it look professional. Many thanks and Regards, Alex Here the fields that the employees should enter: to access the form: an individual number which will be assigned by me that should open the form pre filled with the following data (Note: if the data has not been provided it should be entered by the employee and then be available for next month submission): Name Firstname employee number email phone number yearly private milage allowance the employee then should select: Month and year the submission is for total milage at the end of the month and business milage in that month the database should automatically calculate the difference between the yearly private allowance and the total milage in that month minus the business milage for all months i.e. I want to show the employee how many miles he still has free. y
