Simple 3D model needed for 3D printing and CNCing

Hi,
I am needing a 3D model made of a simple plastic hook. A 2D design showing the front and side view is provided. The images is to scale, also showing measurements (please see attached).
I'd like to 3D print and CNC some of these, so I'll need two files supplied, one ready for 3D printing and one ready for CNCing.
