Wordpress Contact Page Form Design

This is the webpage that I need help with: [login to view URL] The page doesn't have any character and it's not structured the way that I would like it to be. I need the contact form structure more like this web page: [login to view URL] I actually have the HTML code for both of the forms. This will help with the process. Additionally, I want to put the social media Icons in the top right section of the form as well. I would like the form structured in the same way that [login to view URL] is. I would like the right section of the form to be structured in this order: 1. Social Media Icons 2. News Feed 3. Upcoming Events 4. Facebook Post Feed and Likes. I want to follow the same color scheme as the [login to view URL] website, but have the same format at the [login to view URL] webpage. The form background will have to be custom designed, but the form structure should be pretty much the same. Obviously the person that I award this job to must be creative enough to come up with a custom design concept. If you have any questions, please send me a message. I need this job done today!
