Oscommerce checkout after design

Hi, I got an OsCommerce store that uses a one page checkout, and now i want that page design as my attached image. I have also made an workflow on how everything should work in the checkout process, send an message to me and i will provide that and a link to the site. The work includes skills in oscommerce, php, html, css, jquery, ajax and an eye for design! Everything without the discount coupon system is already installed, so will also need an simple coupon addon installed.
