Form layout redesigned - Slick, Intuitive

On our product page,(see attachment) the customer has around 7 different form options available for customizing the product, before adding it to there cart. We need a professional, who is an expert in interface layout. We need the form elements not only to look great, but be in a logical order, intuitive - simple. We welcome CSS, or other methods. But it must be cross-browser friendly. Please do not bid if you are not an expert in this field. You will need to have some php knowledge, since the form and table elements are within some php code. I would like to see some of your form/option interface designs that relate to this project. Please see the attached image for our current (ugly) layout. Good luck!
