Contact Form plus tell a friend

I need a basic contact form with the following features: -A regular contact form which sends me a message with the filled in details when someone has completed the form -A field in the form where the user can add a friend’s email address, an automated message is then sent to that email address inviting the friend to visit the website and also fill in the same form -The message that is sent to the friend’s email should contain the name of the person who invited him/her and the inviters email address, a heading that asks the person to visit the website where the form is, and content text. Also ther is good to be java style plus button that you can add multiple friend mail field. One more think is that when it send the form it dosent reload new site. It jquery the thanks you message to the same div form, like fade the form out and fades text in.
