simple online questionaire / survey with spreadsheet results

Need to install 2 survey / questionnaires (html is ok) to our website.
We will send (non spam) email to up to 200 people inviting them to take the 20 question surveys for our customer service purpose on our site.
The link in the invite email will go to our website where the questionnaire will be hosted.
The results of the questionnaire needs to be compiled in an easy spreadsheet that will list: Name, email, the 20 responses of the questionnaire.
We will need to know who took the survey, who has not and results of each.
I'm sure there is a free program that does tracks the data and maybe even shows graphical data. This will be fine but need this data to show accurately from the survey.
There are 2 seperate questionairs installations in this bid.
