57140 questionnaire scripts

Hi all I need someone to create a script to manage a few questionnaires for me( 3 in total). The html forms have been created I need someone to: 1. create the form validation for each page ie " question has not been filled, when no filled" 2. Create the email from the form entries and also enter entries in mysql database. 3. Create an export as CSV script for all databases. I need to get this done over the weekend (asap) so please only reply if you have the time !!! cheers chewy
