Put hover popup into an includes

We have the attached code that works when we put it on a page as a hover popup. When a person submits, there is a cookie set and the hover stops showing. You can see it work on [login to view URL] We would like you to turn it into an includes that we can put on a different domain so that the hover appears on that domain. E.g. we would put on the other domain that would show the hover. Please confirm cost and how long you can do it. We would want you to create a page on your own domain and show that the includes codes works by showing the hover.
