I need a form placed on my blog (wordpress). You might integrate a form app on Wordpress that meets project requirements or you may need to create standalone app and then just embed on my Wordpress page. I will provide you with all fields required, and the form should also allow users to upload images and documents. I need this form to be added to my website within 8 hours or less. When a person submits the form in the meantime it should either a be automatically forwarded to an e-mail address or placed into a database from which we can view from the admin area (view data submitted) and download files. I need this complete ASAP, and we can work further on some additional backend pieces for the admin area and managing the content submitted by the user.
