Create a preview for a flv video

I have a flv video on the homepage of one of my sites. I need to give it a preview image and playback controls. I tried this myself, but couldn't figure it out.
I will provide the video and preview image. I need a "play" button in the middle and playback controls at the bottom of the video. Basically the same thing you see on Youtube and just about any other sites that have videos.
