website programming/database

The website will be completed in 2 - 3 stages. Stage 1. Design is currently under way. Flash/Html/Css 10 basic pages Programming - not started. this is what we are seeking. Form to be created. It will have a series of fields and based on the data entered will output certain data/results. We currently have a mockup in excel that i will supply to the shortlisted programmers. the Form is designed to take data such as pool size (litres)Location Temperature (c) pool cover (y/n) etc and determine which pool heater is suited. the user will need to enter compulsory fields and then submit the form, the results will then display on screen and all details submitted will be email back to the website owner. the user can edit the form and resubmit if desired. there will also be a clear and a print button. stage 2. form in stage 1 has been created and will be used for users that are not dealers and therfore not logged into the website so stage 2 is mainly the following there will be a admin page to add/remove dealers ( names, adresses etc can be entered) and reset password. on each page there will be a login. email address /password fields. you will need to provide the code for this, our designer will add this to each page. once logged in, when the dealer goes to the form created in stage 2, certain details will be pulled from the when logged in and the dealer goes to the form created in stage 2 the dealer will enter all the details and submit as per stage1. details of this form will be stored in a database for the dealer to retrieve and re-edit later. a form may need to be created for the dealer to retrive the previously submitted forms. as per previous stage - once submitted details will be emailed to store owner. there will also be a clear and a print button. ## Deliverables i imagine that stage 1 is quite simple and that stage 2 is quite complex please quote a price to complete stage 1 and stage 2 separately and also a price to do it all at once. This project will need to be completed as soon as possible I would expect review points and progress reports during creation. 1) Complete and fully-functional working program(s) in executable form as well as complete source code of all work done. 2) Deliverables must be in ready-to-run condition, as follows (depending on the nature of the deliverables): a) For web sites or other server-side deliverables intended to only ever exist in one place in the Buyer's environment--Deliverables must be installed by the Seller in ready-to-run condition in the Buyer's environment. b) For all others including desktop software or software the buyer intends to distribute: A software installation package that will install the software in ready-to-run condition on the platform(s) specified in this bid request. 3) All deliverables will be considered "work made for hire" under U.S. Copyright law. Buyer will receive exclusive and complete copyrights to all work purchased. (No GPL, GNU, 3rd party components, etc. unless all copyright ramifications are explained AND AGREED TO by the buyer on the site per the coder's Seller Legal Agreement). ## Platform Would prefer PHP/MySQL. ASP and Cold Fusion will also be considered. Server platform is currently linux with php support. The website can be moved to another platform to suit whichever programming language is used. ie windows server for asp.
