create a php online-questionnaire

create an online-questionnaire based on 42 questions based on PHP/MySQL. The questions are in german, therefore I need someone who is fit in the german language. The questions are in a word-document. The handling of the questionaire should be a standard questionnaire in the web. For the creation a software-tool specialised for questionnaires can be used, but the result must be a stand-alone php-script that runs un our own server without special licenses. The results of the questionnaire must be saved into a MySQL Database. There a different types of questions: Questions with only one choice, multiple-choice-question and matrices. Some question will be displayed under the precondition of checking options of other questions. The best woulb be a protection of multiple answering of one person, and the possibility of continuing the questionnaire later.
