Using parameterized queries with Reportviewer in C#.NET 2.0

I need a small test application with the following specifications. With it I will also need a manual describing how you used the ReportViewer control to display results from the parameterized query shown below.
1. A C# (DotNet 2.0, Visual Studio 2005) windows application that will connect to an Access 2003 database.
2. I also need you to design an Access database with 3 tables: Table1, Table2 and Table3. Each of these tables will have the same 3 columns: Column1, Column2 and Column3. You may decide the data types for the columns.
3. The C# application must use the ReportViewer control (under the data tab in the toolbox for Visual Studio 2005). Please do NOT use crystal reports.
4. This ReportViewer must be able to display results from a query like this:
SELECT * FROM Table1 UNION ALL SELECT * From Table2 UNION ALL SELECT * FROM Table3 WHERE [login to view URL] = parameter1 AND [login to view URL] = parameter2
5. The parameters will be passed by clicking a button on a form. I'll let you decide how you want to pass the parameters to the report.
I prefer if this can be done by the wizards. If it cannot, code is fine too or a combination of both would also be great. I just need to know how you achieved this.

## Deliverables
1) Complete and fully-functional working program(s) in executable form as well as complete source code of all work done.
2) Deliverables must be in ready-to-run condition, as follows (depending on the nature of the deliverables):

3) All deliverables will be considered "work made for hire" under U.S. Copyright law. Buyer will receive exclusive and complete copyrights to all work purchased. (No GPL, GNU, 3rd party components, etc. unless all copyright ramifications are explained AND AGREED TO by the buyer on the site per the coder's Seller Legal Agreement).
