90730 Contact Box

This is a simple project that shouldn't take the experience programmer long. Don't know that this is PHP, so please forgive if I put it in the wrong category. I need a contact box that comes up to be filled in, very similar to what happens when you add a DVD to your queue on Netflix. I then need this information emailed to me. No database is needed.
