Acrobat PDF Form Design

Please read instructions. Payment will be AFTER satisfactory completion of the project. I will place payment in the Milestone payment. I need this done ASAP. The payment is $30. If you are not willing to do this for $30 then please do not bid. I need 2 PDF's recreated so that I can make edits in Adobe Acrobat. Each PDF is about 4 pages long. I have Adobe Acrobat and just need to have someone build this from scratch in Acrobat. Don't include the logo. We are changing our logo and I will add it or provide it later. The pay is for these two forms recreated in a FULLY EDITABLE format that I will later finalize in Acrobat.. I need this done ASAP
