User Interface to capture Data,integrate into existing code

1. Need to have a form based user interface to capture data in to mysql table. Form has 7 fields with Attachment upload option. Three filed will be dropdown box which will fetch data from backend tables. 2. After Form data is saved into table, the data needs to be displayed in another already developed form as a summary table. If a data is clicked then details editable window will open the form develpoed in step1. 3. This code need to be integrated into an exisitng project which has few modules already developed. 4. Few modules are already present which performs similar task. So it should be an easy task. I would require more such functionality. So I need a person/team with good professional attitude. Need to be flexible with requirement change in the current scope. Happy Bidding :)
