Build a web app for my business

Hello, I am looking for a developer who can help me build a web app. I currently need an app that will allow me to manage returns sent back to my warehouse from customers. I need a form on the front end that will allow my employees to enter important information about the return such as shipping information and damages to the item. Then in the office, I need to be able to access that data and see it in spreadsheet like format with the ability to filter out data, mark as case opened, enter notes, and any other information. The information from the form should sync with my end. I need to be able to open cases with our fulfillment service. The cases should automatically be populated based on the information that the employee in the warehouse fills in based on the form. The form will include fields such as item name, SKU, type of damage, description of damage, and then an option to attach pictures. Currently, I use [login to view URL] as my form site and google sheets as my database to store all the information. I had a program that synced with the google sheets, but now it stopped working. I would like to use a SQL database in back-end and create my own form site. Please let me know what your suggestions are and please submit examples of work. I am interested in having this project completed in 2-3 weeks and will pay a maintenance fee per month for this program. My goal is to add different features to this program, but this is the first step. Seeking a developer that will communicate regularly and keep me posted. Project will be paid when it is complete.
