PHP YOURLS' link shortening system via API on Webpage

I currently have a digital product, that is delivered to the customer via a link authenticated by Username and Password. However, this link is too long of a string, which thus means I need a way to shorten, the authenticated link via YOURLS PHP link. Essentially, you will need to design a webpage, that prompts for a Username and password, that the user enters, which internally substitutes the 'YourUsername' and YourPassword fields within the authenticated link, then relays the complete URL, to YOURLS to shorten the revised link. I will supply access to webpage and YOURLS API Access. In addition, the format of the link will be provided for your referenced. Need this done asap, within a couple of hours if possible.
