Design an A4 Price Sheet

I need an A4, 2 sided price sheet designed.
XLS spreadsheet is attached with the detail that needs to be included.
Template that this is to be design on is also attached as a PDF - same to be used for both pages of the final document.
I require the first two sets of product on one page and last two product sets on the second page.
Only the fonts supplied are to be used.
I am not after anything fancy, just a quick reference sheet for the customer to look at for his pricing.
I do need this to look neat and professional.
Images, fonts and original INDD package will be supplied to successful applicant.
Proofing artwork must be supplied as a PDF.
Final artwork to be supplied as a packaged INDD folder with a high res PDF.
