In your form, I need you to grab the information from the URL parameters and store them as hidden values in the form.
I have a web form on my Wordpress hosted website with several fields like first name, last name, etc. I would like you to create the form and provide me with the code to include on my site.
After the form gets submitted, you should store the lead information in a mysql database. It can be a new database that is separate from the Wordpress install or you can use the same database. Store each field into unique columns in the table. Store the current date and time in the table as well. Store the form submission as 1 entry in the table. It's important that you make it easy for me to add form fields and new URL parameters that ultimately turn into hidden form fields. The way to make it "easy" could be to provide me with step by step instructions on how to add the new fields in the form, revise the form handling code to support the new field, and update the database to store the new field. This way I can make these changes myself in the future if needed.
Submit the lead to a third-party lead buying system called Reply. The SOAP action instructions (use 1.2) are here: [url removed, login to view] - I need someone who has had experience with SOAP action form submissions. Store the response from Reply into the MySQL database and store in a new column associated with the lead stored in the database a few seconds earlier.
