I need to do some customization of this form but the plugin does not come with an easy way to edit this form. Developer suggested I use POEDIT and I purcahsed the pro version but it's out of my depth of understanding. documentation for plugin here: [login to view URL]

the plugin does come with the ability to add fields but the main submission form area needs some adjustments too.
