Create desktop program from VBA coded excel model - 27/04/2018 05:07 EDT

I have an excel model which has been developed using VBA code. The purpose of the "model" is to enter a child's details and get a resulting fee for a childcare facility. There is inbuilt knowledge in the excel formula nd VBA code that once the form is entered the correct bill is generated. What I am wondering is if this model can be transformed into a desktop program using all fo the detail already in the excel so that users do not need excel to create bills. I have attached the excel model so the code and model can be reviewed. If this is possible I would need someone who can turn this around in maximum 2 weeks so please only quote if this is feasible for you once you have reviewed the model.
