PHP and MSQL expert required for creating additional feature on existing job board script

I need someone with Expert PHP and MSQL experience to help me develop an additional functionality to my Job Board script.
Functionality required: Expression of interest online application form.
Description of requirement: Candidates can choose to submit an 'expression of interest' application if they cannot find an advertised job that they are interested in. Expression of interest will be a form where candidates fill in their contact details, attach their CV, and choose their keywords.
Database: The database already has tables in place for these keywords. I will supply documentation with information regarding to these tables and their description.
Note: There is already an application form code int he system, to make things easier, you can adapt the original job application form code and adapt it to the expression of interest form. So what we really need is the keywords. So this should be a simple job.
