Online form with calculated fields.

Hi, I am looking for someone who is able to create a form which calculated cells by what the user inputs and determines whether a client would be approved or not. We are a finance company. I have already built most of the form on the calculated form fields plugin for wordpress but not sure how to integrate it and make it look pretty. My design idea is to have something like this. [login to view URL] But I want that the first few details would be on the homepage slider on my site so that people would be able to access it faster. Also would love to have someone with app development experience as I would like to build an app based on the same form or calculator where people could track their application or apply through the app to get their loans pre-assessed This is an urgent project and only people with previous experience should apply.
