Design A Contact Form with Conditional Logic

I'm okay with webdesign, and could probably figure this out, but I'm out of time and need to complete this project so....it's time to outsource.
I need a simple contact form as lead capture with conditional logic for property management company wordpress site using DIVI Theme from elegant themes. Prefer to use existing theme based contact form, but open to third party plugin such as contact form 7.
Regardless, contact form needs to perform the following functions:
1) Serves as lead capture form (name, email address, and phone number)
AND
2) Based on drop down selection must redirect option 1 to location 1 (third party analysis software company); option 2 send automatic email response #1 and redirect to specified page; option 3 send automatic email response #2 and redirect to specified page.
