Build me a series of Web pages with Forms.

I need a Wordpress Plugin or Theme built. It will consist of a Web page with a series of buttons. Each button will take the user to one of two pages with a form on it. The forms can be filled in by the user and submitted. All entries in the form will then be sent to an email defined by a site admin.
The Plugin/Theme needs to have these features.
1. Be able to be installed on any Wordpress Site as a Plugin or Theme.
2. Able to Edit Background Colours.
3. Able to Edit Logo/Image.
4. Able to Edit Icon/Image Buttons.
5. Able to Edit Form Fields.
