Create a telephone directory application

Create a telephone directory application that saves entries to a single file in MySQL. You should include standard telephone directory fields in the database, such as First Name, Last Name, Address, City, State, Zip, Telephone Number and so on. Create a document as main "Directory". where you can select and retrieve records. Also, Create one document that you can use to add new entries to the database and another document that you can use to edit entries. Overall, I want a page to add new entries as a main page. Other buttons defined in main page are Edit (any of entries in the table of contacts specifically), Search(specific contact), Delete(specific contact), Reset, View(table of contacts order by "Newest","Oldest","A-Z","Z-A") which are linked to their specific pages.
