I can do this project for you. I will design the form and provide a database conection file that you will only have to provide your email you want the form to be sent to and a form layout file that users will enter the information i will then write the script to form the user information into the format you want it and send it your email with that format. Of course i willl have to provide script to validate the user entry.

I have a websites this is an easy job all it takes is a small email script which i could do and it is easy to pull info from a form so thats no ploblem at all. Depending on how much info you need sent this will be a very fast project.
