Create a dynamic employee evaluation module for my website

I need to have a dynamic employee evaluation module for my website that is stored in MYSQL. I am currently using a bootstrap. The template for the bootstrap is [login to view URL] AdminLTE. An evaluation may consist of free text questions, checkbox questions, Dates, Radio buttons. The evaluation Templates are created by the administrators of the site. The administrators will create their questions and possible answer criteria. The evaluations need to be dynamic where the administrators can create the questions and the form type (Text, Select, checkbox, etc). I like this site/editor but would need to have it embedded in my site. [login to view URL] Clients that are accessing it may or may not have access to the internet. The production server is on a LAN. Multiple templates may be created. There may be a template for New employees that are under probation, Template for employees who are just promoted and need to be evaluated quarterly, etc. After the admin creates the templates the supervisors can choose what type of evaluation there are doing. Once completed it needs to be rendered on a screen for the employee to read and emailed to them. Every evaluation that is completed needs to be saved into the database. An employee would be able to see in a datatable all their evaluations and who created them. They can click on it to view it or print it. Once it is completed and “Committed” an evaluation CAN NOT be changed.
