I am looking for a web programming specialist to help me build a web based customer referral portal.

I need to make a basic website portal where our customers can login and refer customers to us.

Once they submit the referral an email will be automatically sent to us and the customers details will be saved to a database.

The portal does not have to look very pretty. It just needs to be functional.

I have attached a more detail brief of what I need.
