Create Email Capture Forms

I need 3 newsletter capture forms for my website. one will be on the page, one will be an exit popup and one scroll in from the bottom. i use the Getresponse system.
I need 3 forms, each with the following placement:
-embedded on the page
-exit popup when the visitor hovers towards closing the window
-slide in bottom corner when visitor scrolls 80% down
please see attached image of forms. one form must have a multiple choice check box where my visitor can select which newsletter they want to subscribe to:
__Kids Classes __Adult Classes
this will assign a tag in my email software which i can later select what content to mail them.
PLEASE NOTE:
i have attached sample forms. please use creativity in designing an attractive and unique form. Pictures can be taken from my website for sample:

we will finalize pictures later.
form must be functioning with my Getresponse account AND for the multiple choice form, the subscriber must be entered into the corresponding email list (one list or both depending on what they click)
i will also like to be able to track which form is performing best with a/b testing.
