Convert MS Access DB to web-based php/mysql with some modifications

You must view the video to understand how the existing database functions. i want it all web-based if possible, and i want to add/modify some features like in the attached document. I was using another freelancer who did an excellent job making this whole database from scratch, and I was going to get him to do all the above work, but unfortunately i'm unable to get a hold of him in the past 2 months, so i'm putting this up for the next developer who will be dealing with us on an ongoing basis. we always require updates and added features, we want someone reliable so we can get a hold of him to continue adding features in the future when needed. as i mentioned, the previous freelancer did a great job, then we requested him to do next updates, then requested him to do 3rd updates, and now we're requesting him to do the 4th updates, but this time no response for over 2 months. developer will be responsible to move all existing client data from this microsoft access database to the new web-based database, this will only be accomplished under my supervision using a remote support software similar to teamviewer, and will have to be done on my computer ONLY. no exceptions because there are a lot of client data which cannot be shared with anyone at all. you must provide an accurate bid price, i'll be available to communicate with all bidders who require more details or clarifications for anything. please keep your bid low, as i mentioned earlier, this will be an ongoing basis, it's not a one-time project. high price bids will be ignored. you must have experience in this type of work, please don't send me testimonials or work you did by making other websites, this has nothing to do with website or web design.
