Clickbank Hoplink Generator Script

We need an HTML form that allows affiliates to enter their Clickbank nickname into the form and then automatically create their entire Clickbank hoplink for our web page. we would insert our page link into the html where directed by you. The final product doesn't need any fancy styling or images...just the form that works. (We're going to copy/paste into our optimize press html snippet to insert into our affiliate page.)
