I need an order form written as a table in 6 columns that list products in rows with various options per product including inputing the quantity and calculating the total price according to which options theyve chosen. The form is submited as an email. I have included a mock up image. Basically the customer will chose the quantity of batteries, a checkbox if they want a bracket and a radio box to chose which plug. When the hover the mouse over the images it should display them in a lightbox. The shipping options should have a checkbox but are not included in the total displayed. If they tick the checkbox for bracket it will add 50 cents which is also multiplied by the quantity of that item.
