PHP script with MySQL backend

Looking to create a PHP script with a mySQL data backend. I have a MS Access database that allows a user to enter records on a single form with a single subform. The Main form has about 40 input fields and the subform has about 15 input fields per record but can have many records.
Once the user has input the values into these 55+ fields, they press a SUBMIT button and the program makes calculations based on their input and some default values contained in a table.
What I'd like to do is recreate the program as a web based application, so they can enter their information press SUBMIT and get an answer.
The project is a bit complicated (although it's only taking this number and multiplying by that number type stuff) but I have NO idea really just how difficult it would be to create. I have basically NO budget to do this as it is just a project for myself and a few friends, but if I can get it done reasonably enough, I am willing to foot the bill.
If this is something you're interested in, please advise what you'd charge for such a project.
