102663 Quote Forms

I have a clien who is a freight broker. I need a quote form with the following fields [login to view URL] Once the user submits a quote it should be stored in a database. The user should be allowed to login and see the status of the quote. The admin can also login and see who requested a quote and answer that quote. Here is how the quote system should work. 1) User requests a quote 2) User and Admin recieve an email stating that a quote was being requested. 3) Admin logs in and answers the request. 4) An email is sent to the requestor that quote request has been answered. If you have something already pre-made please show example. If not, please submit a bid.
