Email in PHP form not working

I have an online form that is suppose to email the recipient the result of that email, but it fails to send the email on "Submit".

This feature worked when the form was 1st developed. The problem was we had to relocate it to another server and are having difficulties figuring out what causes the failure.

If you think this is something you can assist me with that would be great.
