.css for sobi entry form template

1. Need someone, who can apply simple .css styles to the SOBI2 add entry form template. For those unfamiliar with the Joomla component SOBI2, a custom template can be utilized for user's entries. The problem is that the template comes completely unformatted. I'm attaching the sample template so you can get an idea, my template has more fields added. I'm also attaching the exsiting .css so you can see where the modifications need to be made. I'm not looking for anything special, just lining up the columns, and add text formatting using the available elements, like in the demo: [login to view URL] 2. I also need a small amount of php code written, so that certain fields will only be displayed if a checkbox is checked. The majority of this is done, I'm just having trouble with one small detail.
