Create one form, (+MySQL table), and edit another form

This project has three parts: 1. Create a form and MySQL table that collects data from it. The specs for the form are here: [login to view URL] 2. Minor editing to this existing form ([login to view URL]), and MySQL DB. These are the 5 changes that need to be made: [login to view URL] These changes are very simple, they look a bit complicated, but a simple chat will clarify them. 3. Design both forms in a a free jquery template, different and better looking than the existing one. This project needs to be done ASAP. Please do not bid if you didn't read specs, and are not able to do this ASAP.
