Simple Web Based Report

I need of a web page [url removed, login to view] that has multiple forms that when updated will spit the updated fields into some data source (db, excel/csv, xml, txt) from which a second web page called [url removed, login to view] will pull those fields to display the data along with the last time the field was updated.

Mock ups for [url removed, login to view] and [url removed, login to view] are attached.

The files must be saved as htm and it is preferred that they use javascript to perform the data manipulation, but you all can consult me on that piece.
