I'm in a hurry for this project so if someone can get it done fast you would really make my day!! :D This is a simple php project I think, and basically heres what I'm looking for:

There you will find a form.. and basically I need that re-done in a PHP form. I want all the questions to be listed in a table (neatly) and when they answer like "no" to question 1 their score is made +1 and if they answer "yes" their score is made +0 (which is no score).. and if they enter "no" for question 3 their score is made +2.. and so on and so on.. you guys get the idea :) Just add's up the score and displays it after the survey :D
I want the form to go through the questionaire and once its completed and they hit "OK" it tabulates their score and spits out their score and the appropiate reccomendation (1-3, 4-6, 7-8 etc.) in a diff color like red.... something simple like that.. thanks for reading and I hope someone can help me out :)

## Deliverables
1) Complete and fully-functional working program(s) in executable form as well as complete source code of all work done.
2) Deliverables must be in ready-to-run condition, as follows (depending on the nature of the deliverables):
a) For web sites or other server-side deliverables intended to only ever exist in one place in the Buyer's environment--Deliverables must be installed by the Seller in ready-to-run condition in the Buyer's environment.
b) For all others including desktop software or software the buyer intends to distribute: A software installation package that will install the software in ready-to-run condition on the platform(s) specified in this bid request.
3) All deliverables will be considered "work made for hire" under U.S. Copyright law. Buyer will receive exclusive and complete copyrights to all work purchased. (No GPL, GNU, 3rd party components, etc. unless all copyright ramifications are explained AND AGREED TO by the buyer on the site per the coder's Seller Legal Agreement).
