iPad Forms Application - Design only

Hi Freelancers! I require a excel based form to be translated to iPad. At the moment, I just need the design and screenshots of what this could potentially look like on iPad for iOS7. The successful freelancer will need to: 1. Review the excel form, and ask me questions about how the existing excel form works, and what information it captures. The current form is for maintenance of complex equipment, the user is expected to note the location, machine type etc (values are provided) and then capture measurements on the equipment to determine wear. 2. Consider how this will translate to the mobile platform in a concise, easy and intuitive to use manner. UI should be simple and stay out of the way letting the operator do their job. Intended audience for the iPad app are not very technically literate. 3. Provide screenshots of the intended design for me to sell to the client. SCREENSHOTS ONLY - NO CODE WILL NEED TO BE DONE. you can use any iOS UI design tool you have available. Please note that if successful, this design job could potentially lead to building this for the iPad as an additional freelancer project.
