Write an iPhone application

I mobile application will need to capture four pieces of information and write out to a sql server database. The database objects will be generated and developer in-house. The appropriate stored procedure calls will be used within the IOS app to store the data.
Since the database will be within the clients firewal(DMZ), the account credentials will need to be entered once. The IOS application will need to save the credentials for later use.
The application will help capture levels of servics within a health care facility.
